Questionnaire for foreign company’s branch registration

 For this, the following statements and documents need to be  made and provided  by the undersigned:

2-      Type of Company and names of its partners in the case of general partnership and/or mixed company:

3-      Company’s legal address:

4-      Company’s nationality:

5-      Paid- up capital at this date (in hard and local currency):

6-      Company’s registration place, based on its legal documents: 

7-      Registration date:

8-      Authorized official registration body in home country:

9-     Fields of activities in Iran to be conducted by branch in industrial, commercial or financial fields:

10-  Location of the branch in Iran:

11-  Name and address of the company agent in Iran:

12-  Name(s) and address (es) of the person(s) in Iran [other than those mentioned in item 11] who is/are   authorized to receive all announcements to the company:

13-  Name(s) and address(es) of other person(s) who are eligible to sign on  behalf of the company.

14-  A copy of articles of associations of the company, a copy of the power of attorney of agent(s) in Iran, and copy of the company’s latest balance sheet which is prepared according the to the article (7) of implementing regulation of Company Registration law attached to this Questionnaire.  

15-  According to this Questionnaire, I am obliged to hand over a copy of balance sheet of the company to the CRNI office.
